A postcard was mailed with instructions to complete a questionnaire through our on-line system, eJuror. You received a paper questionnaire if (1) We did not receive a response from the postcard that was mailed to you; or (2) You called the jury phone line and left a message, but you did not give the participant number. THIS QUESTIONNAIRE IS NOT A SUMMONS TO REPORT. The questionnaire is used to determine if you are qualified or eligible to serve as a juror in this Court. If you are qualified, you may receive a summons to report for jury service at a later date.
